Every day around 4:00 a.m. PST, you will be receiving an email to your UC Merced email address, and for those that have downloaded the ServiceNow mobile app, you will see an "Emergency Outreach" request in the app.
You are to respond to either the email or the mobile app request, every day.
(If you have not been able to respond to the email or app consistently for the last 14 days, and you are scheduled to be on-site today or in the near future, fill out this attestation form. If you answer "Yes" to the attestation, stay home, and the COVID Response Center will be in contact. If you answer "No" to the attestation, you are cleared to be on-site.)

If you will be responding to the email directly, your process will look like this:
If you are not scheduled to be on-site within the next 14 days, e.g. telecommuting, vacation, etc, simply click the "Not Scheduled to be On-Site" link at the bottom of the email, which will open up an email template where you will then click Send. No further action is required. You will receive an email confirmation of your response.
If you are scheduled to be on-site in the next two weeks, including today, you need to respond to the question: "Have you tested positive for COVID-19 within the last 14 days? or Have you been exposed to someone who has tested positive for COVID-19? or Do you have any of the symptoms listed above?". Fever (100.4F+), Chills, Cough (new), Shortness of breath (not due to chronic condition), Loss of taste or smell, Muscle or body aches (not due to chronic condition), Unexpected fatigue/Severe fatigue, Sore throat (not due to chronic condition), Headache, Diarrhea (not due to chronic condition), Eye redness with or without discharge, Nasal Congestion or runny nose (not due to chronic condition), Nausea or Vomiting.
Your response will either be Yes or No.
If you answer "Yes", stay home and contact the COVID Response Center. You will receive a RED email receipt as shown below.
If you answer "No", you are cleared to be on-site. You will receive a GREEN email receipt as shown below.

About the application
As we carefully and systematically begin to bring some of our campus community members back to campus, it is important that everyone who works on any campus site or in any UC Merced building take precautionary measures to help keep themselves and others well. One such measure is a daily symptom check.
Federal, California and county public health guidelines, UC Consensus Standards and the UC Systemwide Testing and Tracing Task Force Recommendations all advise that individuals should assess themselves each day prior to entering any university workspace or facility. To assist our campus community members in their daily assessment, the UC Merced Health Check application has been developed by the university's Office of Information Technology.
Who Should Use the UC Merced Health Check Application
All UC Merced students, faculty and staff will be required to complete the daily symptom check and respond within the application every day of the week that they are scheduled to work on any campus site or in any UC Merced building.
Description of the Application
The UC Merced Health Check application has been developed by third-party vendor ServiceNow®. UC Merced’s version of the application has been tailored to our audience and needs.
Application Data Management:
The data collected from the UC Merced Health Check application (YES/NO/Not working on-site) will be stored and protected for one (1) year. Then it will be permanently deleted.
The application data does not contain protected health information. Access to the application data is restricted to a small number of UC Merced Human Resources Office (HR) and Academic Personnel Office (APO) personnel and UC Merced Office of Information Technology (OIT) employees specifically involved in supporting the UC Merced Health Check application. Managers, supervisors, other HR/APO personnel and other OIT employees do not have access to the application data. Should this change, you will be notified of the change in advance via email.
UC Merced follows UC System, Merced County, California State Health Department and CDC guidance in response to the COVID-19 pandemic and in recovery efforts. The data collected includes the individual respondent’s name, UC Merced email and work phone number and response selected by day (YES, NO or Not working on-site). As UC Merced progresses through stages of recovery and/or health orders are amended, data retention and handling will be reviewed and may be revised.
