President Drake released the COVID-19 Vaccination Program Policy on July 15, and Chancellor Munoz announced UC Merced’s directive on our mitigation measures on July 16.
 
This week, the implementation guides (see Appendices E & F ) have been released to campuses.
 
In alignment with the policy, all UC Merced employees must submit their documentation or request an exception or deferral by August 4.
For Faculty and Staff
  • Take a photo of, or scan, your COVID-19 vaccination record card.
  • Go to the Mobile-Med website and log in, using Duo Mobile two-factor authentication.
  • Click “COVID Immunization Record” in the left-hand menu.
  • Click “Choose File” and upload your record.
  • Click “Submit.”
 
Accommodation and Exemption Requests
  • Request a religious accommodation, click here .
  • To start the medical and/or disability exception process, a signed certification must be provided as an attachment to your request. The certification form can be found https://risk.ucmerced.edu/covid-19-medical-exemption
  • The medical and/or disability exception request form is in development and is expected to be available next week, by 7/28.
  • The deferral due to pregnancy request form is also in development and will be available by 7/28.
